Please be aware form 01-228 has special instructions:
- Submit completed forms to the Volunteer Resource Center at vrc@gscm.org or by fax to 410-358-9918.
- Indicate if you want a copy sent to you (the requestor on the form.)
- Normal turn around time is a five to seven business days. If you need RUSH service please be sure to indicate that on the form.
- A copy of the certificate will be issued directly to the merchant via fax (if available) otherwise it is mailed directly to the location.
