Certificate of Insurance
Certificate of Insurance for Booth Sale locations (form 01-228)
1. Complete the form and submit it to the Volunteer Resource Center at vrc@gscm.org or fax it to
410-358-9918. 
2. Please be sure to indicate if you would like a copy sent to you as the requestor on the form.
3. Normal turn around time is a five to seven business days, however, given the circumstances if you need RUSH service please be sure to indicate that on the form.
4. A copy of the certificate will be issued directly to the merchant via fax (if available) otherwise it is mailed directly to the location.
